NONIMMIGRANT VISA APPLICATION (three pages)
- For all applicants that have arranged for an interview. Must
be filled out on-line before printing.
Instructions for completing an electronic
visa application form:
1. Enter the information requested into the appropriate spaces
on the form. Please answer all questions. Your answers must
be in English and must use English characters. Letters like
ñ, é, ü, ç are not recognized by the
system. Please enter names like Muñoz and Sémonin
as Munoz and Semonin.
2. Review the information you entered for accuracy.
3. Press the Continue button at the end of the form. Failure
to click on the Continue button will result in an incorrectly
formatted application form.
4. Print the form after it is returned to you and displayed
in Adobe Acrobat. Acrobat
Reader required for printing.
5. The barcode that prints on the last page contains the information
you entered and will help us process your application more quickly.
6. Bring all printed pages with you when you come in for your
interview. Please do not fold the printed application form.
• DS-157 SUPPLEMENTAL NONIMMIGRANT APPLICATION - For all male applicants between 16 and 45 years of age. May be filled out
on-line before printing.

What you need:
• Your Internet browser must support 128-bit encryption.
If you are using Internet Explorer (Windows), the minimum version
that will work with this site is version 5.0, with service pack
2.
• If you are using Netscape, the minimum version that
will work with this site is version 6.2.
• You must also have Adobe Acrobat Reader version 5.0
or higher installed on your PC in order to download and print
the completed application form. If you do not have Adobe Acrobat
Reader version 5.0 or higher, you may download a copy from Adobe's
web site.
• You should use either an ink-jet or laser printer
to print the completed application form
• Form is to be printed on
white, 8 ½ x 11 inches (216 x 279 mm), bond quality, non-glossy
paper.
• Set the PAGE SCALING at NONE and select the AUTO-ROTATE
AND CENTER option, both found under PAGE HANDLING when the Print
dialogue appears.
